Eligibility

The $99 New Patient Special is available to clients of Crew Physical Therapy who have not previously scheduled appointments with the clinic. Participants must be 18 years or older. This promotion cannot be combined with any other discount, promotional offer, or package, unless expressly stated otherwise. 

Offer Details

This promotion consists of (1) evaluation for a new clinic patient, purchased at the discounted rate of $99 ($185 value). 

Booking & Utilization

The purchased evaluation must be scheduled before the end of the purchase month. Clinic staff will contact to schedule your evaluation within 24 hours of purchase (48-72 hours if purchased Friday/Saturday). Appointments are subject to availability, and clients are encouraged to schedule at the earliest opportunity. The evaluation cannot be exchanged for alternative services, products, or clinic credit.

Cancellation & Rescheduling

Crew Physical Therapy’s standard cancellation policy applies to all sessions. Failure to cancel by phone within 48-hours of the appointment may result in a cancellation fee. Please refer to Crew Physical Therapy’s cancellation policy for further details. Sessions may be rescheduled before the end of the purchasing month OR within 7 days of the appointment if the evaluation is scheduled for the end of the month, contingent upon availability. Inability to reschedule may result in the forfeiture of purchase with no refund.

Refunds & Limitations

No refunds are available for the $99 New Patient Evaluation. The session is non-exchangeable. This offer is limited to one purchase and use per new client.

Amendment of Terms

Crew Physical Therapy reserves the right to amend these Terms and Conditions, adjust promotion terms, or discontinue this offer at any time. Changes will be communicated via our website or directly to affected clients as applicable. 

Disclaimer of Liability

Participation in and purchase of the $99 New Patient Evaluation is voluntary, and clients assume full responsibility for any effects or outcomes resulting from services rendered under this promotion. Crew Physical Therapy is not liable for any adverse effects experienced by the client following treatments obtained as part of this promotion.
