By making this purchase, you signify that you have read and agree to these Terms and Conditions of Use. If you do not agree to follow all of the Terms and Conditions of Use, do not purchase.

Eligibility

The BOGO promotion is available to clients of Crew Physical Therapy who do not have future appointments at the start of the promotion and meet all clinic guidelines and requirements. Participants must be 18 years or older. This promotion cannot be combined with any other discount, promotional offer, or package, unless expressly stated otherwise. 

Promotion Details

Clients purchasing one eligible service at the standard rate will receive one additional session of the same service at no charge. This promotion is limited to an (1) evaluation and (1) resulting follow-up session. The complimentary session is strictly non-transferable and valid only for the individual who purchased the initial session. 

Booking & Utilization

Both the paid session and the complimentary session must be scheduled within 45 days from the initial purchase. Appointments are subject to availability, and clients are encouraged to schedule at the earliest opportunity. The complimentary session holds no cash or refund value and is solely for the service associated with the original purchase. The complimentary session cannot be exchanged for alternative services, products, or clinic credit.

Cancellation & Rescheduling

Crew Physical Therapy’s standard cancellation policy applies to all BOGO promotion sessions. Failure to cancel within 48-hours of the appointment may result in a cancellation fee. Please refer to Crew Physical Therapy’s cancellation policy for further details. Sessions may be rescheduled within the 45-day promotional window, contingent upon availability.

Refunds & Limitations

No refunds are available for the BOGO promotion for either the initial evaluation or complimentary follow-up session. Both sessions are non-exchangeable. This BOGO offer is limited to one use or purchase per client.

Amendment of Terms

Crew Physical Therapy reserves the right to amend these Terms and Conditions, adjust promotion terms, or discontinue this offer at any time. Changes will be communicated via our website or directly to affected clients as applicable. 

Disclaimer of Liability

Participation in and purchase of the BOGO promotion is voluntary, and clients assume full responsibility for any effects or outcomes resulting from services rendered under this promotion. Crew Physical Therapy is not liable for any adverse effects experienced by the client following treatments obtained as part of this promotion.
